I would advise you not to alter anything below. The purpose of this form is (1) for you to press the “eyeball” icon above to get to the PREVIEW ID of this form and (2) to link this master form to your Sheet so you can capture scores.

Press the “eyeball” icon above to get to the PREVIEW of this form. Once you press the icon you will copy the unique ID from the address bar. NOT THE WHOLE URL.

The ID is the long string of numbers and letters found between "forms/d/e/" and “/viewform?usp=sf_link.” You want ONLY the numbers, letters, and dashes. Not the "/" or anything else. An ID looks like so (do not use this one, it's just an example): 1FAIpQLScNNYIrlrPszcG9Io2auanSWyZYbbcCsarRHy189fiKJmOHXA

The biggest mistake teachers make is to copy the ID of THIS editable form. That will not work. You must copy the ID of the PREVIEW (again, click the eyeball icon above). Take THAT ID to the QR code maker of your choice and paste it in. You want a "text" QR code, not a weblink etc. 

This site is a good choice for creating a code: https://www.qr-code-generator.com but there are others you might prefer. It really doesn’t matter as long as the output is a text QR code.

You can use your camera or Q Code reading app to see if the code is working. If correct, you will see just the ID appear, and not a link to a website. Your camera may invite you to “search” because it doesn’t really know what else to do with the code. That’s fine…. it means your code is going to work correctly in NoteNames, assuming the text your camera reveals exactly matches the ID of the preview form.

Again, you don't want to copy the ID of this editable master form, you want the ID of the PREVIEW form. In the end the form isn't actually used at all, but its ID will be, so you have to go through these steps. Do NOT send the preview form to students, they will not need it. NoteNames is using the ID to route scores to you.

Now you want to click the Responses tab above and create a linked spreadsheet where all the scores will appear. You definitely don’t want to capture responses here in the form…it’s rather useless.

Finally (essential) you should play a round or two in NoteNames. The pilot of the scoring version is at https://brianwis.com/notescoring/ 

You will notice a cloud-upload icon appears at the bottom of the device. Go ahead and press that and then scan your code using your camera OR choose "Scan an image file" which is a great choice for situations where a camera is not allowed or not available. Now go check your Google Sheet and your score should be there. Good job! Now you can print that QR code and/or save a screen grab of it that can be emailed to students, etc.

Repeat this process for each distinct class (assuming you want them to be routed to their own Sheets). OR, if you want to use the same QR code for all of your classes, you can assign section numbers to students, or they could use their class name (but be aware that typos may abound). You could then sort/filter by "section" as needed. If you haven't learned how to use "query=" it can be really handy for making a tab for each class and grabbing data from the main sheet.

  6. Print/screen grab your QR code
  7. Link this form to a Google Sheet
